Application Procedures

The application process can take several weeks to complete. It is in your interest to begin this process as quickly as possible in order to ensure placement for your child(ren). ALL REQUIRED ITEMS MUST BE INCLUDED BEFORE YOUR FAMILY INTERVIEW CAN BE SCHEDULED.

Required items for application:
1. Copy of birth certificate
2. Copy of current and prior year grade card or transcript
3. Copy of most recent standardized test scores
4. Completed Family Application
5. Complete Student Application for each student
6. Non-refundable registration fee:
If received prior to June 30, $100 per student, $300 max per family
If received after June 30, $125 per student, $375 max per family

Please detach the application form the booklet, keeping the policy sections for your records.

  • a pastoral reference is required. Once all application items are received, we will contact your pastor. Please notify him to respond promptly.
  • After we have received the pastoral reference, a family interview will be scheduled. All custodial parents need to be present.
  • State law requires proof of immunization be on file. Please be ready to turn in documentation of up-to-date vaccinations.
